PHOSA Minimum Requirements – Examples of Leading Practice

This section includes examples of leading practice from the PHO Services Agreement Minimum Requirements self-assessment and peer review process that was undertaken in 2015.

The self-assessment and peer review process was co-developed through a sector working group including members from Networks and DHBs to help Networks demonstrate achievement of their Minimum Requirements under the PHO Services Agreement.

Completed self-assessments were reviewed by Peer Review Panels to provide independent assessment and feedback to help drive business and quality improvement. Organisations that provided the examples have given permission for them to be shared on GPNZ’s website.  These examples may need tailoring to suit your organisation or local region and reflects the collaborative nature of this process.
